INSURANCE FORM

Many major insurance companies now authorize or reimburse the purchase of light therapy equipment for Seasonal Affective Disorder (SAD), sleep disorders, shift work, depression and Circadian Rhythm Disorder. To improve your chances of receiving reimbursement for your light therapy product, send the following documents to your insurance company:

  • Sales Receipt from Aurora Light Solutions
  • Completed Insurance Form OR Your Doctor’s Perscription

We provide an insurance form that lists the correct DSMIV-R code(s) and other supporting evidence for your doctor to complete. The insurance form will make it easier for your insurance company to process your claim. Also, many insurance companies need either a CPT or HCPCS code for the product. This code is used in the healthcare system to label a light therapy box.

Please remember that individual policies vary significantly within the same insurance company. Your ability to get reimbursed, and the percentage of coverage if you are eligible, depends not only on who your insurance company is, but also upon the specific terms of your individual policy.
